1. Use the "Order Sports Hypnosis Program" button below to purchase your custom cd and four hours of personal consultation.
   
2. Send an email to Dr. Grossman with the following information:  Your name, street address and phone number(s). Include a description of what aspects of your game or sport that would like to work on, including a description of your previous experience with hypnosis...if any.
   
3. At this point Dr. Grossman will email you a 100 Question Sports Assessment. Please complete the questionnaire and fax it to us (fax # 1-916-921-1920).
   
4. The next step is to arrange your first telephone interview. This is done by email. During the telephone interview have your questions prepared and call from a place where you will not be disturbed. Expect that the interview will last one hour, however it could take longer.
   
5. After your interview Dr. Grossman will write and record a hypnosis cd for you and send it to you with complete instructions. After you've listened to the cd for at least 10 days, email Dr. Grossman to arrange a time for your second session.
   
6. The second, third and fourth telephone sessions will also last for one hour. Once again please call from a place where you will not be disturbed.
